These forms will help you evaluate your insurance coverage, instructions on calling your insurance to get the needed pre-authorization, and a review of office policies for treatment with us. We are a fee-for-service center and your medical insurance will reimburse you directly within your defined benefits.

• Registration Form

  This registration form will be needed at your first meeting with the doctor. You can complete it and bring it with you in order to save your time in the office.

• Insurance Information Form

  By obtaining this information before your first appointment you will avoid a delay in reimbursement.

• Information Release Form

  Use this form to request and authorize us to release any and all information concerning your medical condition to another provider.
